Download the Points forms (Show and/or Trail) if you wish to participate in the Year-End Awards Program.

Send in the Nomination / Awards Choice form, along with your $10 participation fee, with your first points.
You are encouraged to fill in the information on award preferences, to give the Awards Committee feedback if you win.

Click the links below to download the forms.  Right click and "Save as..." to save a copy directly to your PC.  Or, Left click, and the file will download and open.  Don't forget to print it and/or save it to your PC (using the save icon in the PDF reader, not your browser's file save).
